People with a chronic, physical illness often benefit from regular, systematic check-ups. It has turned out to be difficult to do this at regular doctor’s appointments. This is why the doctors have chosen to entrust this task to specialized practice supporters, called POH’s in Dutch.
Since a couple of years, Rolanda van der Kooi and Yvonne van der Werff have been doing this for our doctors. The doctor refers people to them, and they report back to the doctors.
They see patients for:
- Diabetes type II
- Hart and vascular disease
- Asthma/COPD (specifically by Yvonne van den Werff)
They provide information about the disease, and guide people with the use of their medication and changes in lifestyle. Next to this, they also provide regular check-ups. If need, treatment is adjusted.
Rolanda van der Kooi also visits elderly patients.
The POH’s can also be asked to perform certain examinations to determine vascular health, lung fuction or cognitive function.
Rolanda van der Kooi is present from Monday thru Wednesday, and Thursdays every other week. Yvonne van der Werff is presend on Tuesday, Wednesday and Friday, as well as Thursday mornings.
To get an appointment with the POH, you will have to see the doctor first. Following appointments can be made via the assistants.
